One minute later, inspectors interviewed a nursing assistant who confirmed the cart was indeed unlocked. The aide immediately attempted to secure it after acknowledging the violation.
The facility's wound care nurse also confirmed to inspectors that the treatment cart was unlocked with keys left inside. The nurse was interviewed at 9:09 am, just after the discovery.
More than two hours later, the director of nursing admitted to inspectors that treatment carts should remain locked whenever staff are not present. The DON's interview occurred at 11:28 am, well after the initial discovery of the security lapse.
Federal regulations require nursing homes to maintain accident-free environments and provide adequate supervision to prevent injuries. The unlocked cart violated these safety standards by creating unnecessary hazards for vulnerable residents.
Treatment carts typically contain scissors, needles, medications, and other medical equipment that could cause serious harm if residents with dementia, confusion, or mobility issues gained access. The facility's own policies require securing such equipment when unattended.
