The nurse, identified as RN A, signed off on prescription delivery Rx #327042 on June 3, 2025, according to investigation records reviewed by federal inspectors. In a handwritten letter dated the following day, RN A confirmed receiving the medications at approximately 7:30 PM and wrote that she had stored them in the narcotic lock box.

But loose pills were discovered scattered in medication carts during a November inspection, raising questions about whether residents received their prescribed medications.
"If there were medications at the bottom of the medication cart, then there could be possibility that the resident did not receive the medication," the administrator told inspectors during a November 14 interview. The administrator, who has worked at the facility for 11 months, said nursing staff are responsible for medication audits.
The facility's own policy requires staff to visually inspect, count and sign for all medication deliveries before allowing them into the building. "The staff should not allow medications into the facility unless they have seen it, counted it, and signed off for it," the administrator explained.
When medications that staff signed for can no longer be found, "it could mean the residents did not receive it," he added.
The investigation revealed that RN A passed drug testing and had her own valid prescription for benzodiazepines. But the discovery of loose medications in carts contradicted her written statement that she had properly secured the narcotics in the lock box.
Federal inspectors attempted to interview RN A on November 14 at 11:45 AM but were unsuccessful. They also tried to speak with the hospice company pharmacist who provided the medications at 11:48 AM the same day.
The facility's medication storage policy, dated 2025, states that drugs must be stored "safely, securely, and properly following the manufacturer's recommendations." Only licensed nursing personnel, pharmacy staff or those legally authorized to administer medications should have access to the supply.
The policy also requires immediate removal of "outdated, contaminated, or deteriorated medications and those in containers that are cracked, soiled or without secure closures." Medication storage areas must be kept "clean, well lit, and free of clutter."
But loose pills at the bottom of carts suggest the secure storage requirements weren't followed.
The administrator said the director of nursing, assistant director of nursing and nursing staff share responsibility for medication cart audits. He told inspectors that both facility staff and the hospice pharmacist monitor medications through cart checks, though he couldn't explain why pills were found loose.
The missing medications included controlled substances that require especially careful handling. Ativan is a benzodiazepine used for anxiety and agitation. Morphine is an opioid pain reliever. Haldol is an antipsychotic medication. All three are commonly prescribed for hospice patients but require precise dosing and secure storage.
RN A's handwritten letter described receiving a "total comfort kit" along with the individual medications. Comfort kits typically contain emergency medications for hospice patients experiencing pain, nausea or breathing difficulties.
The investigation focused on a single medication delivery, but the discovery of loose pills suggests broader problems with the facility's medication management system. Federal inspectors classified the violation as causing minimal harm or potential for actual harm to a few residents.
Villa Toscana at Cypress Woods is located at 15015 Cypress Woods Medical Drive in Houston. The facility was inspected following a complaint on November 14, 2025.
The suspended nurse's fate remains unclear as the investigation continues. Meanwhile, residents and families must trust that remaining staff properly handle the controlled substances that many elderly and hospice patients depend on for comfort and pain relief.
