The revelation came during a federal inspection at the 130-bed facility, where investigators found the social service director lacked any of the education or licensing credentials mandated by federal regulations and the facility's own job description.

V12, hired as the full-time Social Service Director on July 19, 2021, had completed only high school education according to her employment record. No college education appeared anywhere in her file.
Federal regulations require nursing homes with more than 120 beds to employ a qualified full-time social worker. The facility's job description for Director of Social Services specified education requirements including a bachelor's degree in psychology or sociology, a Bachelor of Arts or Master of Arts in social work, or a Licensed Clinical Social Worker's Certificate.
The facility had no documentation showing V12 possessed any of these required qualifications.
On January 10, 2022, V12 signed a form acknowledging she had read the job description and "fully understand the requirements set forth therein." By that point, she had been working in the position for nearly six months.
When inspectors interviewed V1, the facility administrator, on October 30, 2025, he expressed uncertainty about his employee's credentials. "He is not sure if V12 has all the required qualifications to work as the facility's Social Service Director," investigators wrote.
The administrator's admission came more than four years after V12 began overseeing social services for all 130 residents at the facility.
V18, the Medical Director, told inspectors on November 3 that he was unaware of the qualification gap. "I was not aware [V12] does not have the qualifications necessary to be the Social Service Director," he said. "You cannot hire someone without the right qualifications. The facility should have followed the regulations and the job description. I did not know this."
The violation affected every resident at Forest View Rehab, according to the inspection report. Federal investigators classified the harm level as minimal but noted the potential for actual harm to residents who depend on qualified social services.
Social service directors in nursing homes coordinate discharge planning, help residents maintain family relationships, assist with emotional and social needs, and serve as advocates for resident rights. The position requires specialized training to address the complex psychosocial needs of elderly and disabled residents.
The facility's own job description outlined the educational standards necessary for the role, yet administrators allowed an unqualified employee to fill the position for years without apparent oversight or verification of credentials.
The inspection occurred in response to a complaint filed against the facility. Investigators reviewed employment records and interviewed key staff members to verify the social service director's qualifications.
Federal data showed the facility maintained a census of 130 residents as of October 24, 2025, placing it squarely within the regulatory requirement for employing a qualified full-time social worker.
The case highlights a fundamental breakdown in the facility's hiring and oversight processes. Despite having written job requirements that clearly specified necessary education and licensing, administrators failed to ensure their social service director met those standards.
V12's employment record contained no evidence of college coursework, social work degrees, or professional licensing that would qualify her for the position under federal regulations. The gap persisted undetected by facility leadership for years.
The Medical Director's surprise at learning about the qualification deficiency suggests a lack of communication and oversight within the facility's management structure. His statement that "the facility should have followed the regulations and the job description" acknowledged the clear violation of established standards.
The administrator's uncertainty about his own employee's qualifications after four years of employment raises questions about the facility's hiring verification procedures and ongoing compliance monitoring.
Forest View Rehab's failure to employ a qualified social worker potentially compromised the quality of psychosocial services provided to all 130 residents, who rely on professional social work expertise for discharge planning, family coordination, and advocacy support.
