Federal inspectors discovered the grooming neglect during a September complaint investigation at Ayden Healthcare of Waterville. The resident, identified as #16, had facial hair approximately one to two inches long on her upper lip and chin "resembling a man's goatee."

The woman has been at the facility since March 2018. Her medical conditions include chronic respiratory failure, quadriplegia, and chronic obstructive pulmonary disease. She requires a tracheostomy and ventilator to breathe.
According to her care assessment, the resident is severely cognitively impaired and completely dependent on staff for all activities of daily living. Her care plan specifically requires "total dependent assistance for all ADL care."
Inspectors observed the resident on September 23 at 11:28 a.m. She was lying in bed, eyes closed and unresponsive. The facial hair on her upper lip hung beyond the lip line. Hair on her chin nearly touched the drainage sponge underneath her tracheostomy, a four-by-four split gauze dressing placed around the breathing opening.
The facility's shower schedule showed the resident was supposed to receive showers twice weekly on Tuesdays and Fridays by night shift staff. Records confirmed she had received a shower on her scheduled day, September 23.
When inspectors returned the next morning, they found the resident clean with freshly washed, damp hair combed to the side. The facial hair remained unchanged.
Certified Nursing Assistant #369 confirmed during an interview that night shift had reported giving the resident a shower. But when asked about the facial hair, the CNA explained why it hadn't been addressed.
"The staff don't want to use the facilities razors on her face so they didn't shave her," CNA #369 told inspectors.
Licensed Practical Nurse #381 was present during the interview and confirmed the extent of the facial hair problem. The LPN stated that staff expectations during showering included trimming nails, washing hair, and shaving facial hair.
Inspectors noted that the facility's razors were traditional two-blade razors.
The facility's own policy on activities of daily living, revised in March 2018, states that residents will receive care and services "to maintain or improve their ability to carry out ADLs." For residents unable to perform these activities independently, the policy promises "the service necessary to maintain good nutrition, grooming, and personal and oral hygiene."
The neglect represents a particular concern for this resident given her medical vulnerability. With quadriplegia, she cannot move her arms or legs. Her severe cognitive impairment means she cannot advocate for herself or request grooming assistance.
The tracheostomy requires careful maintenance to prevent infection and ensure proper breathing. Having facial hair long enough to potentially interfere with the drainage equipment around the breathing opening creates additional risks for someone already dependent on mechanical ventilation.
Federal regulations require nursing homes to provide care and assistance with daily living activities for residents who cannot perform them independently. The citation noted "minimal harm or potential for actual harm" to the resident.
The violation affected one of three residents reviewed for activities of daily living care during the inspection. Ayden Healthcare of Waterville has a census of 75 residents.
The deficiency was investigated under complaint number 2607782, indicating someone had specifically reported concerns about care at the facility that prompted the federal inspection.
The resident remains at the facility, still requiring total assistance for all personal care needs. Her facial hair, according to the inspection report, continued to hang nearly to her tracheostomy equipment even after staff had given her a complete shower and hair washing.
