Inspectors arrived on the morning of September 22, 2025, at 9:15 a.m. and found the facility's posted staffing information showing figures for September 15, 16, 17, and 18. Nothing for that day. They photographed it.
They came back the next morning, September 23, at 9:00 a.m. The board still showed the same four dates from the week before. September 23 was not on it either.
The posting requirement exists for a specific reason: it gives residents, family members, and anyone else walking through the door a way to see, on any given day, how many nurses and aides are actually working. It is one of the few transparency tools available to people who often have no other way to know whether the facility is adequately staffed. A board showing last week's numbers tells them nothing about today.
When an inspector interviewed the administrator that same morning, September 23, around 10:00 a.m., the response was direct. "The expectation is for the staffing to be posted daily by Staffing Coordinator," the administrator said. "That should have been taken care of and updated yesterday."
Yesterday. The administrator's own framing acknowledged the board was already a day behind by September 22, and still hadn't been corrected by the time inspectors returned on September 23.
