"Housekeeping usually comes into her room and only cleans the main area," the resident told the federal inspector on September 16. When the inspector returned the next morning, the dirt and gloves were still there.

The scene repeated itself throughout Lock Haven Rehabilitation and Senior Living during a September complaint inspection. Inspectors found piles of dirt in Unit 4 hallways, trash scattered on floors, sticky substances dried in front of recliners, and buildup around toilet bases that residents said never got cleaned.
Resident 8's room contained crumbs and loose dirt on the floor, with a small pile of crumbs, paper and dirt particles behind the door. Two plastic medication cups sat near the head of her bed. The air conditioner was dirty, the toilet seat was dirty, and built-up dirt surrounded the base behind the toilet.
"They never clean too much and when they do it is just what they can see," Resident 8 told the inspector.
In Resident 14's room, inspectors documented trash on the floor and multiple black sticky spots. Resident 115 had a clear sticky dried liquid substance in front of her recliner, while her floor remained dirty despite daily dust mopping.
"They run the dust mop every day, but they hardly ever scrub the floor," Resident 115 said.
The pattern was consistent across rooms. Resident 5's floor had leftover adhesive from removed non-skid strips near the bathroom, dirty non-skid strips in front of her recliner, and scuff marks by the closet. She told inspectors that housekeeping "most of the time only dust mops the floor and only the part that you can see."
Resident 129 was more direct: "All they ever do is dust mop her room. She said they never mop the floor."
Her room confirmed the complaint. Inspectors found dirty non-skid strips in front of her recliner, beside her bed, and in her bathroom. Loose dirt sat under the bed. Dirt had built up around the base molding and behind the toilet, which was also dirty.
The environmental violations weren't limited to individual rooms. On September 17 at 9:49 AM, inspectors observed piles of dirt in corners and along edges in Unit 4 hallways.
Federal inspectors reviewed their findings with the nursing home administrator and director of nursing on September 17. The facility had been previously cited for similar environmental deficiencies in October 2024.
The inspection also documented water temperature problems, though details were limited in the available narrative.
Lock Haven Rehabilitation operates at 22 Cree Drive in Lock Haven, Pennsylvania. The September 19 complaint inspection resulted in citations for failing to maintain a safe, clean, comfortable and homelike environment under federal nursing home regulations.
