The residents had been offered complimentary haircuts at the facility's hair salon. What happened next, and who made it happen, remained unanswered two months after the facility first learned of the problem.
On July 13, 2025, the day the facility says it became aware of the issue, staff conducted an environmental audit and found a pair of scissors sitting in a resident's room on an adjacent unit. The scissors were removed. That same day, the facility held an emergency quality assurance meeting to discuss what had gone wrong.
The next day, July 14, the Director of Nursing began educating nursing and certified nurse aide staff on residents' rights. Two days after that, on July 16, that education was completed for all nursing staff. The facility's plan of correction was dated that same day.
By the time inspectors arrived in September, Beaumont had already presented its corrective plan and was found to be in past non-compliance, meaning the violations had occurred and the facility had, at least on paper, addressed them. Weekly audits of all residents' hair had been initiated and were set to run for a month. Results were to be reviewed at the next monthly quality assurance meeting, scheduled for October 22.
None of that answered the basic question.
The Director of Nursing said directly to the surveyor: they still did not know who did the haircuts. The administrator confirmed it. A thorough investigation, they said. No answer.
