What staff did document on February 3 was stranger than a blank space. In the field where the weight was supposed to be recorded, a nurse had written "NA," meaning not applicable. A weight ordered by a physician, for a patient under their care, marked as though the question didn't apply.
The surveyor reviewed the full medical record and found nothing, no entry, no note explaining a refusal, no documentation of an attempt that couldn't be completed. The resident's second day of admission also had no weight on file, a gap the surveyor raised separately with the Director of Nursing.
On August 25, at 3:03 in the afternoon, the surveyor sat down with the DON and walked through what the record showed. The DON's response about the January 27 weight: "I don't know what happened to the one on the 27th."
That was the explanation. Four days before the survey closed, the facility's director of nursing had no account of why an ordered weight was never taken or recorded. The surveyor also told her there was no documentation for the weight on the second day of admission. The inspection report does not record any response to that.
Between August 25 and the survey exit on August 29, the facility provided no additional documentation. Nothing was produced to show the weights had been taken. Nothing was produced to show they'd been attempted. The record at the time surveyors left was the same record they'd arrived to find.
Weight monitoring in a rehabilitation setting is not a formality. Physicians order it because weight changes can signal deterioration, dehydration, infection, or the failure of a treatment plan to hold. A resident who is losing weight without anyone noticing is a resident whose care team is flying without instruments. The orders existed because someone making medical decisions for this resident decided the information was necessary. It never arrived.
The "NA" notation is its own problem. Marking an ordered clinical measurement as not applicable is not a documentation error in the ordinary sense. It is a staff member actively characterizing a physician's order as inapplicable, and doing so without any explanation in the record for why. The surveyor's review found no context, no note, no countersigning clinician who had reassessed the order. Just the notation, and then the resident's eventual discharge.
The inspection was prompted by a complaint. The report does not describe what the complaint alleged or who filed it. It describes what surveyors found when they looked: a resident who arrived, stayed for weeks, and left without anyone being able to account for whether the most basic tracking of their physical condition had occurred.
The Director of Nursing knew, as of August 25, that the surveyor had identified the gap. She had four days before the survey closed. No documentation came.
