Alta Vista Rehab: Fall Documentation Failures - TX
A federal complaint inspection completed August 27, 2025 found that the facility failed to accurately capture resident falls on Minimum Data Set assessments, the standardized federal forms that drive care planning decisions for nursing home residents across the country.
The Director of Nursing acknowledged the problem directly to inspectors. She said that if the MDS assessments did not capture a fall, it did not affect care because she or the team would have updated the care plan and put interventions in place anyway. She also told inspectors the missing documentation would not affect payment. It would just mean the fall "was not coded correctly."
That explanation did not satisfy inspectors.
The MDS is not a billing form that facilities can afford to treat as optional paperwork. It is the foundation of the care planning process. The federal Resident Assessment Instrument manual, version 3.0, updated as recently as October 2024, is explicit on this point. Section J of that manual covers fall history, both at admission and at any point during a resident's stay. It directs facilities to use that information to determine whether a resident needs further assessment, rehabilitation services, assistive devices, or changes to their physical environment. It requires facilities to evaluate staffing needs for residents identified as fall risks.
None of that can happen if the fall never makes it into the record.
Alta Vista's own internal policy, dated January 2022, states that residents will be assessed and findings documented in their clinical health record, and that the comprehensive assessment includes completion of the MDS and the Care Area Assessment process. The policy says an accurate comprehensive assessment will include special treatments and procedures.
Inspectors cited the facility under F0641, the federal tag covering accuracy of assessments. The violation was cited at a level of minimal harm or potential for actual harm, affecting a few residents.
The gap between what a Director of Nursing believes is happening informally and what the written record actually captures is exactly where preventable harm tends to accumulate. A care team that knows verbally about a resident's fall history can change shift, turn over, or simply not be present when the next fall occurs. The MDS and the care plan are what remains when memory and familiarity run out.
Alta Vista's director told inspectors the team would have intervened regardless of what the MDS showed. But inspectors reviewing the actual assessment records found the falls were not there.
