Frostburg Rehab Center: MDS Coding Error Found - MD]
The discrepancy surfaced during a complaint inspection completed August 13, 2025. Inspectors reviewed the Minimum Data Set, the standardized assessment document nursing homes must complete for every Medicare and Medicaid resident. The MDS drives care planning, staffing decisions, and reimbursement. What gets recorded there, or doesn't, shapes what a resident receives and what the government pays.
For Resident 5, the form was wrong.
The MDS nurse coordinator, identified in inspection records as Staff 14, told inspectors she is personally responsible for completing Section O of the MDS, the section that captures special treatments and therapies, including respiratory support like BiPAP. In an interview on August 7 at 8:33 in the morning, she confirmed the order had existed since March 31. She confirmed the therapy was not reflected on the MDS. She acknowledged the coding was inaccurate.
BiPAP, or bilevel positive airway pressure, delivers pressurized air to keep a patient's airway open, typically while sleeping. It is prescribed when a resident cannot maintain adequate breathing on their own without assistance. An order for it is not incidental documentation. It reflects a clinical judgment that the resident needs respiratory support.
Whether Resident 5 was actually receiving the therapy during the months it went uncoded, the inspection report does not say. What the report does say is that the official record, the document that follows a resident through their care and informs everyone from nurses to physicians to outside reviewers, said the therapy did not exist.
The MDS coordinator confirmed it did.
CMS rated the deficiency at minimal harm or potential for actual harm, and noted that few residents were affected. Those classifications sit at the lower end of the federal severity scale. They do not mean nothing happened. They mean inspectors found no evidence of serious injury tied directly to the documentation failure, and that the problem was contained rather than widespread.
The facility operates at 1 Kaylor Circle in Frostburg, in western Maryland's Allegany County. The inspection was a complaint survey, meaning someone, a resident, a family member, or a staff member, raised a concern that prompted regulators to come.
Frostburg Rehab Center's provider identification number is 215115. The inspection form, printed April 13, 2026, covers 27 pages. This deficiency appears on page 14.
The MDS nurse coordinator who confirmed the error is still, as of the inspection date, the person responsible for completing the section where the error appeared. The inspection report does not indicate any change in that arrangement.
The plan of correction, if one exists, is not included in the materials reviewed. CMS directs anyone seeking that information to contact the facility or the Maryland state survey agency directly.
What the record shows is a resident with a breathing disorder, a months-old order for respiratory therapy, and an official document that said neither of those things were true.

Data source: This article is based on inspection data downloaded directly from the Centers for Medicare & Medicaid Services (CMS) via Medicare.gov. CMS releases inspection reports in bulk; we publish the findings as documented by state surveyors in the official Form CMS-2567 Statement of Deficiencies.
Plan of correction: The CMS report we receive does not include the facility's plan of correction. Facilities submit plans of correction separately to state survey agencies and those responses may not be reflected in CMS data at the time of publication. The absence of a plan of correction in our data does not mean one was not filed. Readers who want information about corrective steps taken are encouraged to contact the facility directly or their state survey agency.
Corrections may have occurred: Inspection reports reflect conditions observed on the date of the survey. Facilities may have implemented corrections, staffing changes, additional training, or other remediation since the report was issued. We report what CMS provides and encourage readers to seek current information from the facility.
