Lefa Seran SNF: Outdated Vaccine Policy Fails Residents - NV
When federal inspectors arrived at the facility on April 10, 2025, they found a document more than thirteen years old still directing how staff vaccinated residents against pneumococcal disease. The Risk Manager confirmed it herself: the policy was not up to date, and the facility was not vaccinating residents in line with current CDC guidelines. How many residents were affected is listed in the inspection record as "many."
The gap between what the facility's policy described and what the CDC now recommends is not a minor paperwork technicality. It reflects more than a decade of medical guidance that never made it through the door.
The 2012 policy instructed staff to offer pneumococcal immunizations once before age 65 and once after age 65. That was a reasonable summary of guidance at the time. But pneumococcal vaccine science did not stop in 2012. By October 2024, the CDC had published updated timing guidelines that laid out a substantially more detailed schedule, one that accounts for which vaccines a person has already received and when, what chronic health conditions they have, and which newer vaccine formulations, including PCV20, PCV21, and PCV15, are now available and recommended.
Under the current CDC schedule, an adult 50 or older who has never received a pneumococcal vaccine has options the old Lefa Seran policy never contemplated: they can receive PCV20 or PCV21, or they can receive PCV15 followed by PPSV23 at least a year later. Adults who received the older PCV13 vaccine at any age are now supposed to receive PCV20 or PCV21 at least one year afterward. Those who received both PCV13 and PPSV23 after a certain age face a longer wait, at least five years, before receiving the newer conjugate vaccines. Adults between 19 and a specified age with chronic health conditions fall under their own set of recommendations.
None of that complexity appears in Lefa Seran's immunization policy, because none of it existed when the policy was written.
The facility's own stated standard made the failure sharper. The 2012 policy included a line committing the facility to follow current CDC guidelines for immunization schedules. That sentence remained in the document. The CDC guidelines had moved forward. The facility had not.
Inspectors cross-referenced the finding with two other deficiency tags, one related to staff competency and one related to the facility's quality assurance and performance improvement program. The QAPI connection is notable: inspectors found that an unmet goal from a prior surveillance activity had not been reevaluated and carried over into the current improvement plan. The vaccine policy failure was not a new discovery sitting in isolation. It was something the facility's own internal oversight process should have caught and addressed, and did not.
Pneumonia is among the most serious infections nursing home residents face. Older adults and people with chronic conditions are at substantially higher risk of severe illness and death from pneumococcal disease than the general population, which is precisely why the CDC has continued refining its vaccine recommendations as new and more effective formulations have become available. A facility's immunization policy is one of the most basic mechanisms for ensuring residents receive that protection.
At Lefa Seran, the mechanism had not been maintained.
The Risk Manager's confirmation that the policy was inaccurate came during the inspection itself. There is no indication in the report that the gap had been identified or acted on before inspectors arrived and asked about it.
Residents living at 1st and A Street in Hawthorne were, in the meantime, being offered vaccines according to a schedule that predated the current recommendations by more than a decade.

Data source: This article is based on inspection data downloaded directly from the Centers for Medicare & Medicaid Services (CMS) via Medicare.gov. CMS releases inspection reports in bulk; we publish the findings as documented by state surveyors in the official Form CMS-2567 Statement of Deficiencies.
Plan of correction: The CMS report we receive does not include the facility's plan of correction. Facilities submit plans of correction separately to state survey agencies and those responses may not be reflected in CMS data at the time of publication. The absence of a plan of correction in our data does not mean one was not filed. Readers who want information about corrective steps taken are encouraged to contact the facility directly or their state survey agency.
Corrections may have occurred: Inspection reports reflect conditions observed on the date of the survey. Facilities may have implemented corrections, staffing changes, additional training, or other remediation since the report was issued. We report what CMS provides and encourage readers to seek current information from the facility.
Editorial process: Inspection findings are extracted from CMS source documents and synthesized using AI, reviewed for factual accuracy against the original report by our editorial team.
